The HSFC provides FREE medical care for you & your children and Case Management services for your entire family.

Medical Services:

Children & Adult: General medical exams and treatment. ( EAPC Provider-Expanded Access to Primary Care)

Children: Immunization & Physical exams. ( CHDP Provider)

Teens/Adults: Family Planning services & education ( F PACT Provider); STD & HIV screening (confidential & anonymous); Mammogram screening; Male and Female cancer screening.

Counseling: The clinic provides individual, couple, family & group counseling for a variety of psychological problems.

Family Development Network:

FDN program provides FREE services for youth and family assessment.
